Adult Family Care​

Adult Family Care (AFC) is a home-based, long-term care option for eligible Choices for Care participants who need a high level of support but who do not want to live in a nursing home or institutional setting. We work with AFC participants to find out what type of home setting they want and then match them with a contracted shared living provider whose home and lifestyle is a good fit. Participants may choose to live with a single person, a couple, or a family with children. They may choose a home based on proximity to their friends, family, and doctors. Adult Family Care homes provide person-centered support that is safe, supportive, and respectful of the participant’s needs, abilities, preferences, and dignity.
